Thank you for taking the next step toward restoring your marriage. Please review the important details below before completing your enrollment.

1. Complete Required Forms: After you enroll, please visit the Client Forms page to download and complete the Client Intake Form and Informed Consent Document. Both must be completed and signed, then emailed to Dr. Flood directly at DrFlood@maritaltriage.org. These must be on file before counseling can begin.

2. Scheduling Details: Once your first appointment is confirmed, the remaining sessions are automatically scheduled for the same day and time each week.

3. After Scheduling Your Intake: Please disregard any automatic Google Meet link you may receive after scheduling, as it is not HIPAA compliant. Within 24 hours of enrolling, you will receive a secure video session link along with additional instructions and clinical assessments that must be completed prior to your first session.
